Transcripts:
Transcripts should show at least one completed semester of Respiratory Therapy Education. The document should be original but does not have to be official (sealed).

Essay:
The essay should be written in 10-12 point font with no more than double-spacing.

Postmark:
Candidates will only be considered if application documents are postmarked by the deadline date of May 20th. All documents must be included in a single package. Incomplete applications packages will be rejected.

Awards:
The alotment of $250, $500, $1000 awards are not pretermined but will be determined based on statistical distribution methods and will be determined by a point system. Awardees will be determined by committee. The number of awards will be as follows

$250 - 10 scholarships
$500 - 5 scholarships
$1000 - 2 scholarships
